Instructions and Help about Fill Form 5495 Careers

Okay hello my name is Barry Drexler I am a thirty year human resource veteran corporate human resource veteran and I currently manage a company called expert interview coach which you can find online what I'm going to be talking about right now is what forms you as an employee can expect to complete at a typical employer when you are hired for employment pre-employment they're going to ask you to complete an application form make sure it's thorough and make sure you sign it it's okay to put your resume in of the form but it initial in the form that your resume has attached okay then they're going to ask you to complete a waiver and release that form will allow the employer to check references and check your credit and check criminal and so on okay then another pre-employment form before you join is your actual offer letter or your employment contract whichever one applies you need to sign that form to acknowledge that you've actually accepted the job okay now it's your first day that's when the bulk of forms will be needed to complete okay starting off with payroll forms so be ready with how many exemptions you want to put on your w-2 you'll complete federal tax forms state tax forms also you'll be asked to give personal information emergency contacts and so on so be ready with that information also you'll be asked to sign a direct deposit form indicating where you want your paycheck deposited okay that's payroll forms then you'll be required to sign health forms depending on what health plans they offer that would be dental vision life insurance so be prepared with beneficiary information for that for those forms okay and they may offer 401 K you'll be asked to sign up for that plan as well then you're going to have to sign internal policy forms that would be acknowledging that you've read the handbook confidentiality agreements non-compete agreements they these forms vary by company but it's just acknowledging that you read of the policy and those are internal policy forms okay then you're going to be required to come please external government required forms the most important one is the i-9 form and that form confirms that you are able to work in the United States that's a very important form it's the i-9 form you must complete that within three days of employment another government form is a voluntary minority disclosure form where you're asked to voluntarily indicate what your ethnicity is and minority status important as I mentioned is it's voluntary so those are the necessary forms be prepared to complete all those forms and you will do fine Barry Drechsel again thank you
