Now, I'm going to talk about the forms that you, as an employee, can expect to complete when you are hired for employment. Pre-employment, they will ask you to complete an application form. Make sure it's thorough and remember to sign it. It's okay to attach your resume to the form, but make sure to initial that your resume is attached. Next, they will ask you to complete a waiver and release form. This form will allow the employer to check your references, credit, criminal record, and so on. Another pre-employment form you will need to complete before joining is the offer letter or employment contract. Whichever one applies to you, make sure to sign it to acknowledge that you have accepted the job. Now, on your first day, you will need to complete several forms. Starting with payroll forms, be prepared to indicate the number of exemptions you want on your W-2. You will also need to complete federal and state tax forms and provide personal information, emergency contacts, and so on. Additionally, you will need to sign a direct deposit form to indicate where you want your paycheck deposited. Next, you may be required to complete health forms depending on the health plans offered by the company. These forms may include dental, vision, and life insurance. Make sure to have beneficiary information ready for these forms. If the company offers a 401(k) plan, you will also be asked to sign up for it.
